The Breast Reconstruction Procedure
A patient’s breast can be rebuilt using the patient’s own tissue or a breast implant to reform the breast mound and restore a natural appearance and desirable shape. To accomplish this, we use one of two techniques: a tissue expander or flap reconstruction.
Tissue Expansion with a Breast Implant : If you have enough skin tissue, and you are comfortable with using a breast implant, we can use the tissue expansion technique to rebuild your breast. To minimize scarring, we insert a temporary saline implant through the original mastectomy incision, and place it behind the pectoral muscle. Over time, the temporary implant is slowly inflated, allowing your tissue to gradually expand. A more permanent breast implant will replace the temporary implant once an optimal pocket size is achieved. Once the permanent implant has been placed, and tissue has fully expanded, Dr. Kobienia can reconstruct the nipple and areola.
Flap Reconstruction : A patient’s own tissue can be used to recreate the breast if there is insufficient tissue at the reconstruction site, or if the patient does not wish to use an implant. The flap reconstruction technique rebuilds the breast using muscle and skin tissue from the thighs, buttocks, back or abdomen. This tissue remains connected to blood vessels, while it is repositioned and reshaped at the breast area to create a new bust line. In other cases, extra skin tissue and fat from the lower abdominal area can be used to rebuild a breast. This is called a TRAM (transverse rectus abdominis myocutaneous) flap.
As with tissue expansion, the nipple and areola can be constructed once the initial reconstruction has healed.
Flap Reconstruction with Implants : Depending upon the patient’s goals for breast reconstruction surgery, implants can be used in conjunction with the flap reconstruction technique.
Breast reconstruction is an impatient procedure requiring one or two nights of hospital recovery, especially if it is performed immediately following a mastectomy. Full recovery can take several months, but most patients can resume normal activity in about a week or two following surgery.
